System interfaces (HL7 and DICOM)

MedStar uses Health Level Seven (HL7) clinical and administrative software standards to provide for the exchange, management and integration of data that support clinical patient care and the management, delivery and evaluation of healthcare services. Specifically, the standard is used to create flexible, cost effective approaches, guidelines, methodologies, and related services for interoperability between healthcare information systems.
Our HL7 adheres to a strict and well-defined set of operating procedures to insure compatibility and uniformity. The most widely used aspect of our HL7 is a messaging standard that enables disparate healthcare applications to exchange key sets of clinical and administrative data, especially demographic and order data, between MedStar and other systems and equipment within a medical practice, hospital setting, a Radiology practice or other medical facility. MedStar uses HL7 to interface to:
Hospital Information Systems (HIS)
Picture Archiving Systems (PACS)
Electronic Medical Records Systems (EMR)
Other healthcare systems
Our DICOM solution provides DICOM messaging capabilities between the MedStar RIS and individual imaging modalities. The DICOM software provides configuration options for modalities to communicate with the trusted remote DICOM server. The DICOM solution is installed and licensed per database.

Example - MedStar Systems Interface Offering: HL7 to a Third Party EMR software
MedStar Systems offers an HL7 interface between its Practice Management System and the PrognoCIS Electronic Medical Record system (EMR).
The HL7 Interface specification for PrognoCIS is as follows:-
- Export new /updated patient details from MedStar System's practice management system to PrognoCIS
- Export new/reschedule/cancel/modify medical appointments from MedStar's practice management system to PrognoCIS
The HL7 Interface specification for Medstar Systems practice management system is as follows:-
- Import charge capture from PrognoCIS
MedStar Systems interfaces the HL7 messages via TCP/IP sockets.
PrognoCIS interfaces HL7 messages using files.
The solution is HIPAA compliant
In this example, Medstar Systems practice management software runs on a server that has secured FTP client software. MedStar provides patient scheduling capabilities. The PrognoCIS EMR runs on a separate FTP server.
The Scheduler in the FTP voyager is configured to monitor 3 different folders which are named as patient, appt and charge in the MedStar server. When a new patient is registered or an existing patient's details are updated in the Practice Management Software, an HL7 file record will be created for the new patient. The scheduler automatically uploads these files to the PrognoCIS server and deletes them from the MedStar Systems Server.
Similarly, for new, reschedule, cancel and modify appointments activities, corresponding HL7 files are created in the appt folder and the files are uploaded to PrognoCIS server.
When charge capture is employed, the scheduler is configured to monitor a folder in PrognoCIS. Once the charge capture file is created in PrognoCIS, the scheduler automatically downloads the file to the charge folder in the MedStar Systems Server.

Segment Specification
Patient Export
The Segments to be sent for a new and updated patient are as follows: MSH, PID, NK1, PV1.
- MSH -
The time of creation of the file, sender, receiver and the type of message details are specified in the MSH segment.

PID -
The segment contains data related to patient like Patient name, address, and phone number, account number, date of birth, patient marital status.

PV1 -
The segment contains Primary Physician details the patient has registered to and in addition to that referring physician details if required.

NK1 -
The NK1 segment contains patient emergency contact and Next of kin contact information, each contact information will be sent in a separate NK1 segment.
Appointment Export
The Segments to be sent for a new, reschedule, cancel and modify appointments are as follows: MSH, PID, SCH, PV1

Charge Capture
A service developed in .net will be running as a system process. The service monitors the charge folder in Medstar Server for new files, any new charge capture file downloaded from PrognoCIS to this folder, the Service reads the contents of the file and stores the required fields into the Medstar Systems Practice Management System Database.
